Three years after the explosive events of the first film, the immortal warrior Andy and her team must face a terrifying new threat—one of their own.
The movie opens with a sweeping shot of ancient ruins in the Middle East. A shadowy figure walks across the desert—Quynh (Veronica Ngo), once thought lost to the ocean, now free, powerful, and changed.
While Andy (Charlize Theron) and Nile (KiKi Layne) work with Booker (Matthias Schoenaerts) to take down illegal biotech experiments in Africa, a secret global faction known as The Reclamation rises. Led by a wealthy immortal named Dr. Selene Vorta (Uma Thurman), The Reclamation believes immortality should no longer be hidden—but weaponized.
Quynh has not forgiven Andy for leaving her to rot in the depths of the ocean for centuries. Tortured by madness and solitude, she emerges with a chilling calm and a new ideology:

Quynh allies with The Reclamation, becoming its deadliest enforcer. With her guidance, they capture and reverse-engineer the DNA of fallen immortals, unlocking a serum that grants temporary immortality — a power Selene intends to sell to the highest bidder.
Andy’s team regroups in Marrakesh after a devastating ambush leaves Nile wounded and nearly killed. They realize Quynh is alive—and now an enemy. Andy is conflicted. Guilt over losing her immortality (from the first film) weighs heavy, but Nile’s rise as a leader gives her hope.
Joe and Nicky (Marwan Kenzari and Luca Marinelli) track Reclamation mercenaries across Europe, discovering a chilling truth: Selene plans to use the temporary immortality serum on a private army to seize control of major world governments through assassination and infiltration.
Still seeking forgiveness for his past betrayal, Booker sacrifices his safety to infiltrate Selene’s base in Iceland. There, he discovers a prison of failed immortals—men and women given temporary life only to die in agony once the serum wears off. Horrified, he sends intel to Andy before he’s captured by Quynh.
Quynh doesn’t kill him. Instead, she whispers: “You know what loneliness is, Sebastian. I offer you a way out. Side with me… or suffer for eternity.”
The climax unfolds in Vienna, where Selene hosts a secret auction to sell the final formula of the immortality serum to world powers. As politicians and warlords gather in an underground fortress, The Old Guard launches their final assault.
Andy, knowing she can die, leads from the front. Nile fights like a woman possessed, and Joe and Nicky unleash their full strength, vowing never to lose another family member. The fight is brutal—tight corridors, burning labs, shattering glass.
Booker escapes his prison and helps turn the tide.
In a heart-wrenching duel beneath the fortress, Andy confronts Quynh. Their battle is not just physical—it’s emotional, poetic, and devastating. Quynh accuses Andy of giving up on hope, of betraying their shared dream. Andy pleads with her:
“We were supposed to save the world. Not destroy it.”
Quynh hesitates—until Selene orders her to kill Andy. In a moment of clarity, Quynh turns her blade on Selene, ending her reign. But the damage is done. Explosions rock the base. Quynh is mortally wounded.
Andy cradles her in the rubble as Quynh finally finds peace. “You came back,” she whispers. “Too late… but you came.”
The film ends with the team in hiding. Andy, no longer immortal, chooses to walk away from the battlefield, passing the leadership torch to Nile.
Nile forms a new code: “We don’t fight for revenge. We fight for those who never had a chance.”
Joe and Nicky adopt young war orphans they saved in Sudan. Booker, seeking redemption, begins teaching others like them how to live in the shadows.
In a post-credit scene, a mysterious figure arrives at an ancient temple. He opens a scroll, revealing more names of immortals yet to be awakened.
