In the year 2045, after decades of global warming and war, cities are pushed underground to escape the poisoned atmosphere above. One such city is Neo-Seoul, a vast subterranean megacity where life thrives within layered metro zones. Each layer is connected by a sprawling train network known as the Z-Line, controlled by the all-powerful MetroCore corporation.
But deep in the abandoned tunnels of Zone Z — once sealed due to a biological outbreak — something awakens.
A new strain of the undead, more intelligent and faster than any before, begins to spread. When a train carrying diplomats and civilians is rerouted into Zone Z during a system malfunction, all communication is lost.
The film follows Dante Raynor (John Boyega), a former Metro Security operative turned train mechanic, now living in the lower sectors. Haunted by the death of his wife during the Zone Z outbreak years ago, Dante lives off the grid—until he’s forced back into action when a distress signal comes from a train lost in Zone Z.
The passengers include Dr. Elara Voss (Ana de Armas), a geneticist who once worked on the virus program known as Project Lazarus, and Lana (Millie Bobby Brown), a teenage stowaway with an immunity to the infection.
As Dante assembles a small team of ex-military fighters and hackers, including the stoic samurai-cyborg Shin Takeda (Hiroyuki Sanada), they board an old metro engine and descend into the forbidden depths.
Zone Z is unlike any other part of the city. Trains sit derailed and rusting. Lights flicker endlessly. Strange sounds echo through the tunnels. Then the team sees it: not just zombies, but "smart infected" — creatures that mimic human behavior, set traps, and even control parts of the metro grid.
Dr. Voss reveals the horrifying truth: Project Lazarus was not a virus, but a failed immortality serum meant to extend human life using engineered parasites. The infected are the experiment’s aftermath — and the lost passengers are now hosts.
As Dante and his team fight through ambushes and collapsing tunnels, Lana begins showing signs of strange abilities. Not only is she immune — she can sense the infected. She may be the key to stopping the virus, or… the final stage of its evolution.
The rescue train reaches Terminal 0, an abandoned station buried beneath the original city’s ruins. There, the team faces the “Hive Alpha” — the original infected subject of Project Lazarus, a grotesque fusion of machine and parasite. It controls the horde via neural signals through the rail system.
In a brutal final battle, Shin Takeda sacrifices himself to destroy the main hive conduit. Dante confronts his past, overcoming his fear and rage to lead the civilians to safety. Lana, under threat of transformation, uses her immunity to release a pulse of engineered antibodies, temporarily neutralizing the infection.
Only a few survive. The upper city seals Zone Z once again, denying the outbreak ever happened. Dante and Lana disappear into the underground, now fugitives. But as trains roll again and people forget, a final shot shows an infected child hiding aboard a departing metro…
The Metro Z virus is not gone. It's just waiting for the next stop.
