In Hellboy 4: Rise of the Blood Queen, the demonic anti-hero returns in his most apocalyptic and personal battle yet. Picking up years after the fall of Nimue, the Blood Queen, the world has entered a fragile age of magical awakening. Supernatural events have begun tearing through the fabric of Earth — portals to the underworld are opening, monsters long buried are rising, and ancient prophecies whisper of a “Crimson Rebirth.”

Hellboy (David Harbour), now exiled from the Bureau for Paranormal Research and Defense (B.P.R.D.), is wandering Europe, haunted by visions of destruction — with himself at the center. Despite his gruff demeanor, he is tormented by a growing sense that he may not be Earth’s savior, but its destroyer.

Everything changes when Alice Monaghan (Sophie Okonedo), a powerful seer and Hellboy's longtime ally, finds him and brings dark news: Nimue’s soul was never destroyed — only scattered across dimensions. A secret cult called The Crimson Thorn has begun resurrecting her in pieces, using forbidden rituals to rebuild her body from blood and shadow.

Milla Jovovich returns in stunning form as Nimue, the resurrected Blood Queen, more powerful than ever and no longer bound by the old rules of magic. Her plan is no longer conquest — it’s transformation. She wishes to merge the worlds of men and monsters into one eternal kingdom of chaos, with Hellboy by her side as her king.

But Nimue has a secret: she and Hellboy share a deeper, older connection than even he realizes. She claims they are bound by ancient prophecy — that he was born not to defeat her, but to complete her.

Reuniting with Professor Bruttenholm’s spirit through a forbidden séance, Hellboy seeks guidance from his father figure one last time. Meanwhile, he joins forces with a reformed B.P.R.D. team, including the amphibious Abe Sapien (played by Doug Jones in a surprise return) and a mysterious exorcist monk named Brother Varek, who carries a cursed book that may hold the key to Nimue’s destruction — or Hellboy’s downfall.

As Hellboy battles grotesque monsters across ruined temples in Romania, haunted forests in Ireland, and the ghost-riddled catacombs of Paris, the line between man and beast, hero and weapon, begins to blur.

In a dramatic twist, it’s revealed that Hellboy’s “Right Hand of Doom” is not just a key — it is a lock. Nimue’s full resurrection is only possible if Hellboy embraces his destiny and willingly unlocks the final gate of Pandemonium, the capital of Hell. As Nimue tempts him with visions of peace for monsters, and humanity’s extinction, Hellboy is forced to confront his identity: is he a man raised by humans, or the heir to hell?

The final battle takes place on the cliffs of the ruined Stonehenge, where time and magic collapse into chaos. Swarms of demons rise from the ground, B.P.R.D. agents clash with ancient beasts, and Hellboy must choose between saving the world or ruling it.

Rejecting both prophecy and fate, Hellboy chooses his own path — using his infernal powers not to open the gate to Hell, but to seal it forever. In doing so, he sacrifices a part of himself, burning away the demonic fire that gave him strength. Nimue, betrayed and weakened, is dragged back into the void she tried to escape.

In the aftermath, Hellboy is left mortal, scarred, and changed — no longer a weapon, but a man. As dawn breaks over the battlefield, Alice tells him: “Maybe it was never about what you were born to be, but who you chose to become.”

 

Deep beneath the ruins of the B.P.R.D., a relic glows — an ancient egg begins to crack. A single glowing eye opens inside.
